A Study on the Needs of the Chronic Mentally Ill Patients in a City



Abstract
The Purpose of this study was to investigate the needs of 64 chronic mentally ill patients registered at the Public Health Office in a City and to utilize the basic data to develop the community mental health program for chronic mentally ill atients in the community.
The subjects select only 64 out of 73 chronic mentally ill patients (64 home- resident, except 9 in-patients) who registrated at Seogu public health office in a city. The data were collected from September 3, 2001 to February 23, 2002 and analysed by Number, Percentage, Mean and Standard Deviation, t-test, ANOVA, Using the SPSS 11.0 Program. The results were as follows.
General Characteristics of the subjects were 37.5% males, 62.5% females. In terms of age group, 74.8% were over 36 years. And 34.4% were single, 37.5% married/live together, 28.1% separate/divorced and bereaved. 24.4% were under middle school graduate, 43.8% high school graduate, 7.8% college graduate. In terms of religion group, 50.0% of the subjects were christian, 18.8% were the others, 31.2%nothing. 1.6% had their house, 25.0% reservations, 60.8% lease, and 12.6% the others. 17.2% of the subjects had occupation, 82.8% had no occupation. 15.6% of the subjects had not live together, 29.7% lived one member, 31.3% lived two members, 15.6% lived three members, and 7.8% lived over four members. 92.3% no monthly income. 79.6% thought that their income was not very enough or not enough. 66.8% had no the others with meaning, 12.5% had one, 6.2% had two, 12.5% had over three, 31.3% of the subjects had friend with psychiatric disease, 68.7% had nothing.
Psychiatric characteristics of the subjects were 42.2% had psychiatric disease under 9 years, 57.8% over 10 years, and then 37.5% of the subjects had a admission, 26.5% had 2-3 times, 35.9% over 4 times. 89.1% of the subjects had under 3 months for admission duration in the last two years, 4.7% 4-12 months, 6.3% over 12 months, 58.4% of the subjects had the diagnosis of the schizophrenia, 40.6% the others.
The economic ability of chronic mentally ill patients¡¯ needs were the score of 2.34, sexual life 2.20, physical health 2.12, thinking problem 2.01, social relation 2.03, information of medicine 2.00, residence problem 1.85, safe for self 1.84, illiteracy 1.48, safe for the others 1.39 and psychedelic drug 1.17. The average of needs were the score of 1.74(minimum 1.00, maximum 3.00), and items over the score of 2.00 were economic ability, sexual life, psychological health, physical health, information of medicine, social relation and management of personal hygiene.
As a result of comparison of needs-level for chronic mentally ill patients¡¯ general characteristics, with sex group there was significant difference with respect to needs of drinking problem(t=2.324, p=.023), supporting children(t=2.111, p=.039), using the telephone(t=-2.771, p=.027), with medicare group there was significant difference with respect to needs of residence problem(t=-2.972, p=.004), housework (t=-2.022, p=.047), and social relation(t=-2.318, p=.024), with friends with psychiatric disease group there was significant difference with respect to needs of sexual life(t= -2133, p=.037).
In terms of GAF score group, 6.2% of the subjects were under the score of 40, 12.5% score of 41-50, and average GAF score of the subjects were 61.8. With distri- bution of GAF score there was significant difference with respect to needs of housework(F=2.691, p=.023) and sexual life(F=2.806, p=.046).
In summary, the chronic mentally ill patients were a large number of needs for social problem as a sexual life, social relation, psychological loneliness, aloneness, depression more than it for clinical problem.
Therefore, it is required that the chronic mentally ill patients are offered systematic job-rehabilitation program and safe work for their needs and functions. Also, they are serviced weekly support program and home visiting program for social need in public health office or mental health service center.
